Ready to Book Your Group Tattoo?

Thanks for choosing me for your tattoo. Please read the info below before submitting.

IMPORTANT! This form is for GROUP TATTOO appointments ONLY. If you are not in a group, use the personal (1 person) booking form.


How It Works

  1. Choose a group leader. All communications will be with the group leader only.

  2. Each group member fills the form carefully.

  3. Each person must fill out the form with their individual email address. Do NOT submit multiple forms with the same email address.

  4. I will email the group leader the quote and a link to book your appointment.

  5. Your group leader pays the deposit to lock in your spot.

  6. Your deposit goes toward your final tattoo price.


Important Info Before You Book

  • All booking is done online using my calendar link.

  • A deposit is required to hold your appointment.

  • Deposits are refundable if you cancel 2 days (48 hours) before your appointment.

  • Deposits are NOT refunded if you cancel less than 48 hours before your appointment.

  • No call / no show: There is a $25 rescheduling fee to book again.

  • Groups: Each person must submit their own form. Be sure to include your group leader's name on the form so I can keep everyone together.


Minors (Age 16+)

I tattoo minors ages 16 and up with parent or legal guardian consent. You must bring:

  • Your photo IDIf you don’t have a government photo ID, bring a school ID or any ID with your name and photo together.

  • Your birth certificate

  • Your parent/guardian’s photo ID

  • Your parent/guardian must be present and sign in person
